I don't know how big Blackmoor's dungeons are supposed to be, but if you assume that nobody has gotten to the bottom, add in some extra levels and then scoop out the middle (where the crater is) whatever is left could include tunnels that have been there for thousands and thousands of years.
Even with radioactivity (if you use that sort of thing) or disease, there could be underground nations that have been there before Blackmoor was built up above them.
