1d4 for spiritual hammer damage is wrong AFAIK. I don't have the books at hand at the moment 'cause I'm at work, but it should be 1d6 IIRC. Also, the cleric attacks as if being four levels higher than his actual level with it. Sounds a bit better, doesn't it? Given the fact that it's a mere 2nd level spell, it should be pretty useless for a level 16 cleric because of his many other possibilities at that level. I'm aware that 1E isn't always stringent with that rule (magic missile is the counter example for it as one of the few overpowered spells of that edition), but in general it works that way.

It's been too long since I played OD&D myself (more than 30 years in fact!), so I don't remember the rules very well anymore. Those old B and X modules are fantastic though, and I still play them today occasionally, converted to 1st edition.

1d4, 1d6 not much difference. It's 1d6 for man-sized and 1d4 for larger creatures, iirc.
It's true that a lv 16 cleric has more options, but has much less of them. The bulk of his spells is still low-level, and due to the way spells are granted, it will continue to be always. Same with other magic users.

But there's no point in arguing about the rules =)

I miss Wraithform in 3.5. It is a really high level spell now called something else! [Forget what to be exact]
