St Carmichael
#1
I just noticed St Carmichael mentioned in Fred Weining's Archbarony of Blackmoor article. AFAIK St. Carmichael first appeared in Mike Carr's module B1 - In Search of the Unknown. Since St. Carmichael is an obvious reference to the author, I was wondering if we could link him to Mike's Blackmoor PC, Bishop Carr? Are there any other references to St. Carmichael in Greyhawk or other D&D material?

I don't think St. Carmichael was ever mentioned again in any official source. Tal Meta developed him in this PDF document as one of three saints serving the Greyhawk god Rao, each representing an aspect of Rao's sphere of influence (in Tal Meta's version, St. Carmichael is the god of peace).
